Objective. To develop a personalized method for calculating the time to reach a certain coagulation activity threshold after the administration of a selected preventive dose of factor VIII concentrate and compare the results of the calculations with the data of a population pharmacokinetic study in hemophilia according to the WAPPS-Hemo.
Materials and methods. After signing the informed consent at the Center of the Pediatric Oncology, Hematology and Immunology 20 boys aged between 12 and 17.5 years with severe congenital hemophilia A (basic activity of factor VIII less than 1%), a pharmacokinetic study was performed to calculate the frequency of administration and amount of the preventive dose of the drug «Octanate».
Results. The method for personalized calculating of the time to reach a certain coagulation activity threshold after the administration of a preventive dose of FVIII concentrate was developed. The results of personalized calculations of pharmacokinetic parameters, having significantly less spread of values and are closer to reality than similar population values calculated using the WAPPS-Hemo service.
The most similar data to the results obtained by the personalized method were the results of pharmacokinetic studies estimated by the “conservative” version of WAPPS-Hemo calculations. The using of population pharmacokinetic data should be carried out according to the results of personalized calculations based on real pharmacokinetic studies.
