DIAGNOSIS OF CORONARY HEART DISEASE IN PATIENTS WITH TYPE 2 DIABETES

Objective. To carry out a comparative evaluation of the ultrasound diagnosis results and to assess their value in diagnosing coronary heart disease (CHD) in patients with type 2 diabetes.

Materials and methods. The study included 150 patients who were under care of Republican Research Center for Radiation Medicine and Human Ecology SI with a verified diagnosis of compensated or decompensated non-insulin-dependent diabetes mellitus (NIDDM) and verified diagnosis of coronary heart disease: stable angina on exertion of functional class II—III. Those persons formed the main study group. The control group consisted of 30 healthy patients.

Results. Presence of type 2 diabetes in patients with ischemic heart disease was found to lead to more significant changes in the cardiovascular system structural-and-functional state manifested in decreasing of the coronary reserve, reducing of myocardial deformation (p<0.001), thickening of the intima-media complex of the common carotid arteries (p<0.001) and dysregulating of the blood vessels endothelial-dependent vasodilation (p<0.001). Speckle-tracking echocardiography was determined to be diagnostically accurate, highly sensitive, and informative for diagnosing cHD in diabetic persons.
