SELF-MONITORING OF BLOOD PRESSURE USING WRIST DEVICE: NEW OPPORTUNITIES

Objective. To compare the BP levels and the hemodynamic phenotype determined by the QARBPP method in independent measurements using automatic devices on wrist (W) and on upper arm (A).

Materials and methods. Totally, 89 normotensive young people (23±0.2 years old) determined their BP values sequentially by an automatic W tonometer and by an A tonometer at the office twice a day until the 22—25 BP values were obtained for each of the devices. These individual BP series were used for the hemodynamic phenotypes defining by QARBPP; a comparative analysis of the results obtained by the W and A devices was carried out.

Results. No statistically significant difference in the BP values measured using the W and A tonometers except for
a higher pulse pressure measured by the W tonometer (p=0.015) was identified. The distributions of the hemodynamic phenotypes diagnosed by both the A and W measurements (proportion of individuals, %) were comparable (p>0.05). The harmonic phenotype prevailed — 80.9% and 78.8% for the A and W measurements, respectively. QARBPP according to the BP series obtained with the W tonometer makes also possible identify the “hypertensive” and dysfunctional normotensive phenotypes with acceptable accuracy identifying the category of individuals with increased cardiovascular risk.

Conclusion. Use of a wrist device provides adequate BP values not differing significantly from those obtained by an upper arm device and in case of the BP self-monitoring allows determine the hemodynamic phenotype by QARBPP with an acceptable accuracy. Advanced wrist devices in conjunction with QARBPP have prospects in the preventive and sports medicine as well as in the BP self-monitoring in hypertension patients.
