A retrospective analysis of the medical records of inpatients with coronavirus infection COVID-19 with the development of pneumonia (n=30), hospitalized in the clinic of the State Institution «Republican Scientific Practical Center of Pulmonology and Phthisiology» from April 1 to July 15, 2020 was carried out.
The risk factors for the development of an unfavorable outcome of COVID-19 include the presence of multiple chronic somatic diseases, obesity and bilateral lungs damage in patients. One of the main markers associated with lethal outcomes were: high levels of C-reactive protein over 100 mg/L, serum ferritin over 1000 ng/ml and D-dimer over 1000 ng / ml.
A feature of morphological manifestations was diffuse alveolar lung injury with microangiopathies, which are based on injury the vascular endothelium of the lung and less often other organs by a virus with the development of SARS-CoV-2 associated endotheliitis.
In connection with the leading role of lesions of the microvasculature of the lungs, hypercoagulable syndrome with thrombosis of the vessels of the lungs and other organs in the pathogenesis of COVID-19, a preventive approach to anticoagulant therapy is required.
