SOME FEATURES OF DIAGNOSIS AND PERIOPERATIVE MANAGEMENT OF PATIENTS WITH INTESTINAL STRICTURES IN CROHN’S DISEASE

Objective. To study some features of diagnosis, prognosis, surgical treatment and perioperative management of patients with the abdominal variant of the stricturing form of Crohn’s disease of the small and large intestines.

Materials and methods. An analysis of treatment was made of 50 patients with a stricturing form of the abdominal variant of Crohn’s disease, examined and operated on in 2016—2022 at the Minsk Regional Clinical Hospital.

Results. For the first time, an analysis of the comparison of the sensitivity of classical enterography and CT enterography for small bowel strictures did not show significant differences. Statistical relationship between the presence of postoperative complications and the level of C-reactive protein, leukocytes before and after surgery was not found. An increase in the incidence of complications was found in patients with albumin levels less than 26 g/l and total protein less than 51 g/l. The POSSUM prognostic scale showed a moderate correlation with actual complications. The parameters for determining the boundaries of resection of the small intestine are noted. A direct relationship was found between the length and diameter of the stricture.

Conclusion. The data obtained are important for the development of recommendations for the diagnosis and surgical treatment of patients with the stricturing form of Crohn’s disease.
